Transfer of Care Liaison Nurse- Grade 5

Role details

We are seeking a highly motivated Nurse to work alongside FNHC adult services teams to support the safe transfer of care in and out of FNHC services.

Your career to date will reflect your passion for community nursing. You will have extensive knowledge and experience of the challenges and opportunities in delivering nursing care in people’s homes and managing this safely.

The successful candidate will have knowledge and experience in managing risk and be comfortable with healthy professional challenge.

The post holder will develop excellent working relationships with the MDT across Jersey so as to ensure excellent communication, planning  and joined up working to ensure that the patients move across care settings safely.

You will work in partnership with patients and their families to ensure that their voice is heard in discharge planning and that they are empowered to work in partnership

We welcome applicants who are dynamic and innovative with excellent communication and organisational skills.  You will be able to demonstrate leadership skills, have the ability to work within a changing environment and be flexible and keen to develop to meet service needs.

Applicants will be required to demonstrate a strong evidence base in community care and a commitment to working with the wider multi disciplinary team to deliver positive outcomes for patients in the community.
