Grade 5 Registered Nurse – Rapid Response and Reablement Team (RRRT)

This role will support the delivery and development of a specialist service for a clients with a defined set of conditions involving nursing interventions and the provision of advice within a multi-disciplinary framework. This will involve developing and maintaining a high clinical profile and presence and acting as role model for other team members. You will work in partnership with patients, their carers and family (where appropriate) to and the wider MDT to support the patients’ health and wellbeing

hours: 37.5 pw - Applicants must have flexibility to work shifts over a 7 day week

salary: Grade 5 - £66,423 - £71,038 depending on experience

criteria: You will be a highly experienced practitioner in order to contribute to the development of a clinical nursing service within a multi-disciplinary team. You will be educated to a nursing degree level, experience of working as an autonomous practitioner and working with adults with long-term conditions. Own transport is essential.

This is an exciting opportunity to join our professional and forward thinking team within Adult Services at Family Nursing and Home Care (FNHC).

The successful candidate will be enthusiastic about collaborative working and committed to be self-reflective.
Clinical responsibilities include –

Being able to demonstrate specialist competence, innovation and clinical leadership in the assessment, intervention and delivery of community nursing in all contexts appropriate to individual needs within the RRRT caseload. This requires:

The ability to undertake complex assessments using a variety of assessment tools and consultation models appropriate to the situation and application of physical and clinical examination skills to inform the objective assessment encompassing all aspects of the patient’s needs.

Advanced communication skills that encompass the ability to respond appropriately to patients in heightened emotional states without immediate recourse to backup. This includes skills of influencing and negotiation, both for patients and staff.

Effective multidisciplinary and multi-agency team working, alongside the ability to work independently and accept professional accountability and responsibility and supporting and developing the service

Competence in the delivery and co-ordination of evidence-informed, person-centred and negotiated care across the age spectrum.

Adaptability in working in unpredictable environments, autonomous decision-making, effective communication strategies for staff, patients, families and carers and use of technology to improve care management and patient self-care.

Strong emphasis on supporting patient self-care and management and tailored health promotion and evidence-informed behaviour change strategies, ability to signpost patients and carers to suitable resources for their needs and supporting and developing this approach in other staff.

Role modelling and embodiment of non-judgemental, value-based care encompassing the 6 Cs in practice and expectation and promotion of these values in other team members.

Anticipatory care to be embedded in practice and utilisation and ongoing development of care pathways aiming for hospital admission avoidance and achievement of preferred place of care for patients.

Skilled use of technology to support independence and patient self-care at home to improve self-care and reduce exacerbations.

Competence in working in partnership with patients with a wide range of acute conditions, long-term conditions and acute exacerbations of these and those patients needing palliative and terminal care.
