Health Visitor

Role details

This is an exciting opportunity to join our professional Health Visitor Team who form part of our extensive Child and Family Services team.
You will be flexible, enthusiastic, have an ability to manage change and be motivated to work as part of a team as well as an autonomous practitioner.

The post holder will deliver and develop a specialist service for a group of clients with a defined set of conditions involving technical interventions and the provision of advice within a multi-disciplinary framework. This will involve developing and maintaining a high clinical profile and presence and acting as role model for other nursing staff.
You will be a highly experienced practitioner within the multi-disciplinary team and will contribute to the development of a clinical service through involvement in standard setting and the development and dissemination of expertise within both the clinical and education settings.

Key Accountabilities:

Nursing Practice

The post holder will promote a culture of evidence-based practice by contribution to developing and maintaining protocols, documentation, systems, standards, policies and guidelines in line with the Clinical Governance Framework.
You will demonstrate enhanced nursing practice and will act as a role model in terms of own care delivery by acquiring knowledge and skills to support a client-based service supported by the multi-disciplinary team. The post holder will support individual clients/carers through all aspects of treatment programmes, providing a ready means of access and advice.

Safeguarding Children and Vulnerable Adults

You will take responsibility for the safeguarding and welfare of all children, adults and/or families within their care and conduct and must be familiar with the signs and symptoms of abuse and know what to do if any such concerns are raised or are identified. You shall be aware of and comply with Family Nursing & Home Care safeguarding and policies and procedures. This will include being conversant with Children’s Law (Jersey) 2022, Jersey Children First and the Team Around the Child approach to practice.

Team Management, Building and Working

The post holder will work collaboratively as a member of the multi-disciplinary team, taking the lead role when appropriate and participate in building team commitment, coherence and spirit.

Resource Management

The post holder will contribute to the overall aims of the service by developing the nursing staff and encouraging self-sufficiency and flexibility of working practices to provide the most cost-effective and efficient working practices. You will contribute to the Business plan for the specialty service and will be responsible for budget management, ensuring cost effective use of resources.
