Team Lead – Rapid Response and Reablement Team (RRRT)

The FNHC Rapid Response and Reablement service provides nursing and therapist support within a community setting; in the islander’s home, place of residence or clinical setting. This service is available to any individual with a clinical need that the team can support with a short term intervention. This intervention would typically last from 72 hours to a maximum of six weeks. The role includes operational management of community-based services, effective use of resources, awareness of budgetary responsibilities, review of clinical standards and ongoing development of evidence-based practice. You will be expected to play a proactive role in developing the relevant services through contribution to overall service strategy

hours: 37.5 pw - Applicants must have flexibility to work shifts over a 7 day week

salary: Grade 6 - £75,921 - £80,684 depending on experience

criteria: Essentially you will be a Registered Nurse, educated to Degree Level 6 with 3 years relevant practice and proven supervisory/assessor experience. You will be working towards or willing to work towards a Master's Level 7 in a health-related qualification e.g. Advanced Clinical Practice (ACP) and Independent Prescribing award V300. It is essential that you have your own transport.

Role details

We are looking for an exceptional person to work with and lead our Rapid Response and Reablement Team.

The aims of this professional service are to:
Avoid a hospital admission,
Support and facilitate hospital discharges and care transfers into the community,
Avoid premature long term admission to care homes,
Provide cost effective care and the right services the first time,
Provide brief and timely support to care homes,

The Rapid Response and Reablement service offers three different levels of service which covers:

Acute illness
Crisis management
Reablement
To deliver these services, FNHC Rapid Response and Reablement work closely with GPs, the hospital and other healthcare providers as a multidisciplinary team. The team is made up of highly skilled professionals including Advanced Clinical Practitioners, Mental Health Nurses, Senior Community Nurses, Physiotherapists, Occupational Therapists, Social Workers, Therapy Support Workers and a variety of other support workers. This service is designed to respond promptly to acute, crisis and reablement episodes of care which do not need hospital admission.

This role demands, amongst other Key Accountabilities :
Professional Leadership
• The post holder will demonstrate a thorough knowledge of the guidelines of the NMC and Code of Conduct and Scope of Professional Practice and apply these to maintain trust and credibility with patients, carers and colleagues.
• The post holder will make significant contribution to service direction by demonstrating vision and ability to assist in the direction and leadership of their team in an effective and efficient way.
• The post holder will balance clinical/managerial responsibilities, adopting a management style/structure which proves to be effective in performing both aspects of the role.
• The post holder will keep fully up to date in developments within their own professional discipline and will share clinical knowledge/experience in order to improve nursing care, acting as a role model and thought leader for professional practice.
• The post holder will maintain a professional portfolio which demonstrates the continuing acquisition of skills, knowledge, attitudes, understanding and achievement.

Team Management, Building and Working
• You will be responsible for building team commitment and coherence. They will build on existing team spirit by demonstrating the ability to inspire, motivate and lead.
• The post holder will ensure that all team members understand the service objectives and their roles and responsibilities in achieving these.
• You will develop and maintain collaborative working relationships and will encourage cross boundary working.
