Tissue Viability Practitioner – Part Time

The Tissue Viability (TV) Practitioner role is a clinical, hands-on supportive role working in collaboration with the TV CNS. The Tissue Viability Practitioner delivers specialist clinical expertise in the prevention and management of wounds, pressure ulcers, and skin integrity issues. They work collaboratively with all teams within the organisation: across adults and child and family services and colleagues in the wider community (for example: Care homes/care agencies/GP Practices) to optimise patient outcomes and embed best practice. The post holder will have a pivotal role in meeting the psychological, emotional and information needs of patients with complex wound management issues, their families and significant others. He/she will be a key member of the inter-disciplinary clinical team responsible for the care of this patient group with the support of the TV CNS.

hours: 22.5 per week

salary: Grade 5 - Ranging from £33.95ph - £36.30ph depending on experience

criteria: Essentially you will be a Registered Nurse with a minimum of 2 years' experience in your area of practice - with an in depth knowledge and a keen interest of Tissue Viability issues. Own transport is essential

Role details

The role of the Tissue Viability Practitioner is to ensure that his/her knowledge in complex wound management are utilised effectively and efficiently to facilitate the provision of an equitable and quality patient focused service that enhances the health status of the population.

The TV practitioner will liaise closely with all clinicians involved in the management of patients who require complex wound management. He/she will be a key member of the inter-disciplinary clinical team responsible for the care of this patient group.
Specific duties include but are not limited to :
• Pressure Ulcer Root Cause Analysis (PURCA) Panel
• Leg Lounge
• Team/TV clinics for complex reviews – complex being wounds or patient circumstances (complex wound as defined in the Wound & Lower limb Management Framework)
• Team clinical support meetings
• Checking requests and arranging orders for compression garments
• Service development – identifying gaps in service delivery and developing suitable TV provision for the island.
• Governance: quality and safety initiatives , education delivery and competency review , risk management , support policy development and audit
• Equipment reviews
• Pressure mapping and liaison with external agencies re queries concerning care delivery, use of assessment tools and other TV related issues.
