Trainee Reablement Support Worker (TRSW)

This role is based within our professional and forward-thinking Rapid, Response and Reablement Team (RRRT). The successful candidate will provide guided reablement therapy, personal care, monitoring, assisting with continuous healthcare checklists, nursing interventions and support to patients within their own home. The post holder will follow the support plan and work daily with the person receiving the reablement service. The focus is on observing, guiding and encouraging the person who uses services to do things themselves, to rebuild confidence and improve skills that may have been lost. The service will provide short intensive time limited services to maximise independence and promote independent living.

hours: 37.5 pw - Applicants must have flexibility to work shifts over a 7 day week

salary: Nursing Grade 2 during training (Grade 3 on completion of of Assistant Practitioner Foundation Degree (therapy) & full Competency sign off Reablement Support Worker

criteria: Essentially you will have a minimum of 3 years as a Senior Health Care Assistant in a Community Nursing Service & hold RQF/QCF Level 3 or equivalent qualification . You must be willing to complete the Assistant Practitioner foundation degree (Nursing) and meet the competencies required within 2 years of commencement of the programme.

Role details

This is an exciting opportunity to commence a training post with formal Assistant Practitioner training foundation degree (therapy) and practical training using a competency framework. The aim is to achieve RSW competency within 2 years or commencement of the programme and will be supported by regular reviews and supervision.

Key Accountabilities are:

• To participate in MDT management of the RRRT caseload and working to agreed care and treatment objectives.

• Delivery of guided therapy and nursing interventions as part of a multidisciplinary team to patient within an agreed scope of practice.

• To ensure all patient documentation is written in accordance with professional standards.

• To participate in delivery of patient care and treatment. Meeting and reporting on progress and any areas of concern following intervention to the relevant member of staff.

• To support joint home visits with RRRT staff and to be able to independently complete home visits.

• To issue agreed prescribed equipment to patient property and complete appropriate documentation.

• Once appropriate competence has been achieved issue a limited range of equipment independently and seek supervision when indicated.

• To deliver prescribed mobility and home exercise programmes.

• To undertake specified rehabilitation programmes without direct supervision across the 7-day working week.

• Completion of incident and risk management processes.

• Successful completion of core competencies assessed by registered staff.
