Role details
The successful applicant will follow the support plan and work with the person receiving the reablement service. The focus is on observing, guiding and encouraging the person who uses services to do things themselves, to rebuild confidence and improve skills that may have been lost. The service will provide short intensive time limited services to maximise independence and promote independent living.
You will have previous experience of a physiotherapy and occupational therapy role with knowledge and understanding of long-term health conditions.

Key Accountabilities include:
• To participate in MDT management of the RRRT caseload and working to agreed care and treatment objectives, liaising with colleagues as appropriate to provide a safe and efficient therapy service.
• To liaise with external agencies under the guidance of senior staff which includes changes to homes to meet patient needs, external relevant referrals and to establish ongoing support when needed.
• To establish a rapport with patients and to provide education, ongoing support and encouragement throughout their treatment programme to achieve their agreed SMART goal objectives.
• To ensure all patient documentation, records of assessment, intervention and progress to monitor effectiveness, is recorded in accordance with FNHC standards. Report to the appropriate therapist on the patient’s performance, thus contributing to the evaluation of the patient’s SMART goals.
• To facilitate the progress of patients to help achieve identified SMART goals using Physiotherapy/Occupational therapy/Nursing skills, knowledge and equipment. Meeting and reporting on progress and any areas of concern following intervention to the relevant member of staff.
• To have a basic knowledge of anatomy and physiology relating to patients in a specialised area and understand normal movement patterns to assess patients’ mobility and transfers. To provide teaching on the safe use of walking aids to maintain the patient’s independence and deliver prescribed mobility and home exercise programmes.
• To support joint home visits with RRRT team members and to be able to independently undertake specified rehabilitation programmes without direct supervision across the 7-day working week.
• To issue and supply delegated, agreed prescribed equipment to patient property, and complete appropriate documentation, checking it is safe and fit for the patient’s need.
• To identify when assistive equipment may be needed or need adjusting and liaise with therapist.
• To monitor and order stock items to facilitate effective and efficient running of the service.
• Completion of incident and risk management processes.
• To regularly carry out established data collection methods as required and assist in audit and analysis of statistics and patient outcomes to provide evidence for the effectiveness of therapy input.
• To fulfil the requirements of FNHC mandatory training and comply with the standard performance appraisal system.
